Junior Physicals
Click on "Sports Physicals" or "Junior Physicals" for appropriate paperwork to print out.
The Pennsylvania Interscholastic Athletic Association (PIAA) requires all students to have one sports physical conducted per school year. PAHS offers multiple opportunities throughout each school year for a student to obtain a sports physical. Appropriate paperwork needs to be completed in order for clearance.
-
If this is the first sport (this school year) your child plans on participating in they will need to complete a CIPPE packet which is NOW sections 1-6 (page 6 is the doctor's physical page).
-
Each subsequent sport (in the same school year) does not require an additional physical but additional paperwork (section 7 form).
-
If at any point in a sports season, your child seeks treatment from a provider for injury or they are removed from play your child must have a section 8 form completed by a physician.
-
This form may be completed by a doctor DO or MD ONLY.

The state of Pennsylvania requires all 11th graders to have a physical before the completion of their junior year. Sports physicals do count towards this requirement if you choose to do so. Otherwise, the school doctor, Dr. Jason Tronetti, will be in the building several times during the school year to help meet this requirement. You may also see your own private physician.

***PLEASE NOTE THAT A SCHOOL PHYSICAL & A SPORTS PHYSICAL IS NOT MEANT TO REPLACE A WELL-CHILD EXAM WITH YOUR PRIMARY CARE PHYSICIAN***
