OTC and Prescribed Medications

Medication Administration

EVERY medication that needs to be taken in school must have a provider's signature, a parent/guardian signature and must be validated by the school nurse before the medication will be administered by the school nurse. 

  • If medication is prescribed by a provider, or is not one of the listed OTC medications, the medication must be brought in BY AN ADULT, and a provider's signature and parent/guardian must be on file for EACH MEDICATION TO BE ADMINISTERED. You should sign the form and take the form to your doctor. You can have your provider fax the form directly to the school. (MAKE SURE THE PARENT HAS ALSO SIGNED). CLICK HERE FOR THIS FORM.

  • If the medication is classified as OTC (such as Tylenol, cough drops) the appropriate forms must be completed in advance. For the 2024-2025 school year we only require a parent confirmation because our school doctor has already provided us with an agreement. Please note that as long as your child is in enrolled in our district, we only need your confirmation for each medication you want your student to receive throughout the school day. (Please click the black button above for this form). We have made signing OTC paperwork easier for you, by requesting that you complete this form online. By completing the form online, the school nurse receives the paperwork digitally, instantly. This will reduce traffic into the nurse's office, cut down on lost paperwork, and provide instant confirmation of permitted medications.

  • Please note that students in grades 7 through 12 are permitted to carry their inhaler and epi-pen directly on them.
